By submitting this registration form, I give permission for my child to participate in Light of Christ’s (LOC) Vacation Bible School (VBS). I understand that I assume all risks related to my child’s participation in all activities of LOC’s VBS. It is agreed that LOC, nor its staff/volunteers do not assume any legal or financial liability for injuries or other losses from participation. In the event of a medical emergency, and parents/guardians cannot be reached, I do hereby authorize adult staff/volunteers of LOC to consent to any medical care deemed necessary.
